If you need to speak with a nurse or doctor outside of these hours, or when the office is closed, please call this office. Elliot Health System is committed to full and open disclosure of our relationships with other health care organizations. Please be aware that Elliot Senior Health Primary Care Physicians serve patients in several of the extended care facilities in the local area. A list is available upon request from the patient’s Elliot Hospital case manager or by calling Elliot Senior Health Primary Care (603) 663-7030. If you do choose one of the facilities that is covered by the Elliot Senior Health Primary Care Physicians, please inform the staff upon admission to the extended care facility if you would like to be followed by our physicians. You have the right to specify your physician choice to the facility’s staff. |

Monday through Friday 8:00 a.m. to 5:00 p.m. Weekend After Hours ClinicElliot Physician Network primary care providers recently opened a “Weekend After-Hours Clinic” on Saturdays and Sundays from 8 a.m. to 2 p.m. at Briarwood Primary Care (located near Elliot Hospital at 445 Cypress Street, Suite 5 in Manchester), allowing their patients the flexibility of being seen for acute matters over the weekend. A rotating schedule of on-call EPN providers will determine which doctor a patient sees at the clinic. If necessary, the physician on-call will follow up with the patient’s primary care provider. Patients access the clinic by calling their physician’s office. The Nurse or the on-call provider will discuss the patient’s symptoms and make a referral to the After-Hours Clinic. |
