Pediatric Asthma Project

This population includes EPN patients under age 18 with a diagnosis of asthma. Our overall goal is to improve chronic disease care and influenza vaccination rates by improving education and follow up care in children with Asthma.  Specifically we are looking to increase the following rates:

  • % Patients with diagnosis of Asthma having 2 office visits within 12 months
  • % Patients with diagnosis of Asthma having influenza vaccination during the Influenza season

Definition   Patients age < 18 with a diagnosis of either Asthma or COPD having at least 2 office visits within 12 previous months

Denominator   Number of patients with a diagnosis of Asthma in their EPICare problem list
