Vaccine Information Sheet Granite State Medical Center is an Internal Medical office, which has been providing healthcare in Manchester for more than 10 years. Our physician, Wieslaw Walczak M.D. is Board Certified. Our friendly, knowledgeable staff provides you with the professional services that you deserve. Our office is currently accepting new patients ages 17 and above and would appreciate the opportunity to serve your healthcare needs. Dr. Walczak has been practicing medicine in Manchester since 1994. He completed his internship and residency in Internal Medicine at the University of Connecticut and has an advanced degree in Medical Sciences. He began practicing medicine in Europe in 1971. He had practices in Poland and West Germany, where he specialized in Internal Medicine and Non-Invasive Cardiology and was also involved in teaching and research. Office Hours: Monday, Tuesday, Wednesday, and Friday 8:00 a.m. to 5:00 p.m. Granite State Medical Center has physician coverage 24 hours a day.
Weekend After Hours Clinic Elliot Physician Network primary care providers recently opened a “Weekend After-Hours Clinic” on Saturdays from 10 a.m. to 2 p.m. at Briarwood Primary Care (located near Elliot Hospital at 445 Cypress Street, Suite 5 in Manchester), allowing their patients the flexibility of being seen for acute matters over the weekend. A rotating schedule of on-call EPN providers will determine which doctor a patient sees at the clinic. If necessary, the physician on-call will follow up with the patient’s primary care provider. Patients access the clinic by calling their physician’s office. The Nurse or the on-call provider will discuss the patient’s symptoms and make a referral to the After-Hours Clinic.

All patients new to our practice are asked to fill out a Pre-Registration Form, which is sent via U.S. Mail in their new patient information packet, and can also be downloaded and printed from this site. Please click here to download the Pre-Registration Form. Directions From Elm Street go onto Valley Street. Drive up Valley Street until you come to the intersection of lights on Massabesic Street and Tarrytown Road. Take left on Tarrytown Road; take second left to Doctor's Park and our office is the first building on the right.
From Mammoth Road, take Nelson Street to end which brings you onto Tarrytown Road. Take your second left to Doctor's Park, and our office is the first building on the right.

What Insurance do you accept? Our practice, as well as all other Elliot Physician Network locations, participates with most insurance companies. We can provide you with a list of participating insurance entities. You may also click here for a complete listing of the insurances accepted at Elliot Physician Network offices.
How long does it take to obtain my test results? You should receive a letter or phone call within two weeks of having your test. This will give the physician time to receive and review the test result.
