Nursing at the Elliot

Nursing at the Elliot is both an art and a science.  Elliot nurses provide care and comfort to patients, families, and the community in a wide variety of settings.  Our team of nursing professionals is comprised of highly skilled and dedicated clinicians, many of whom hold advanced credentials and certifications in their areas of expertise.  Elliot nurses collaborate with other healthcare professionals to care for the whole patient.

Our specialized nurses work in a variety of medical and surgical departments, treating inpatients as well as outpatients.  The Visiting Nurse Association provides care in the patient’s home.  Elliot’s nursing team also includes licensed practical nurses, licensed nursing assistants and technicians.  All members of the Elliot nursing staff receive specialized education tailored to meet the needs of their patients.

The Elliot Model of Nursing

The Elliot Model of Nursing was developed from the collective experience of Elliot nurses. Four emerging themes - caring, competence, teamwork and continuity - are integrated with the following definition to describe the work of the Elliot nurse:

 

“… to assist the individual, sick or well, in the performance of those activities contributing to health or its recovery (or to a peaceful death) that he would perform unaided if he had the necessary strength, will or knowledge.  And to do this in such a way as to help him gain independence as rapidly as possible.”

Caring:  A critical characteristic of nursing is caring, which includes meeting the physical, emotional, and spiritual needs of each patient.  Elliot nurses recognize the individual needs and goals of the patient, and are sensitive to cultural and value differences.

Competence:  The nurse integrates knowledge, skill, experience and confidence to interact in the patient’s best interest.  

Teamwork:  The nurse partners with others at all levels, across departments, disciplines and settings to achieve positive patient outcomes and a positive patient/family experience.  Patients, families and the community are integral members of the team.

Continuity:  Elliot nurses ensure a smooth transition from shift to shift and between departments through comprehensive and effective communication.  There is a continuous flow of information that connects the patient/family with caregivers along the continuum of care.

Intensive Care Unit

The Intensive Care Unit, located on the second floor, is a 14-bed critical care unit. All rooms are private and have glass doors, allowing nurses to keep a close watch on patients at all times. 

 

The Intensive Care Unit cares for a variety of patients (pediatric, adult, geriatric) and treats a wide range of diagnoses including: trauma, cardiac, respiratory, neurological and surgical.  Critical care nursing is based on the Elliot Model of Nursing (Caring, Competence, Continuity and Teamwork). The nurse to patient ratio is 1:2, we use a multidisciplinary team approach to patient care.  This team includes:  physician, nurse, pharmacist, respiratory therapist, dietician, social worker, and rehabilitation therapist.

The Intensive Care Unit is committed to improving the health and well being of our patient population by providing specialized medical and nursing care by a highly trained staff utilizing the most current technological equipment available.  Care is delivered in a compassionate manner.  We affirm individual worth and dignity by addressing the needs of the body, mind and spirit.  We are committed to maintain the highest level of critical care expertise in order to meet the diverse needs and exceed the expectations of our community.  We take pride in our collaborative relationships with our patients, their families and all ancillary services that are involved in returning our patients to health and wellness.

We are committed to the highest standards of quality and excellence.  We show respect for each other with open and honest communication and work collaboratively to create an atmosphere of healing.  We encourage patient autonomy and independence and we take pride in the success of others.


Visiting

The Intensive Care Unit operates 24 hours a day, 7 days a week.  Visiting hours are 24 hours a day and are collaboratively decided upon by patient, nurse and family taking into account patient condition and needs.  Children under age 14 are permitted to visit when supervised by an adult visitor.

The Fitch Unit

Fitch Unit is located on the sixth floor of the Elliot Hospital.  Our comfortable unit has well-designed and equipped rooms and has the capacity to accommodate a total of 41 patients within its three designated clusters: outpatients, acute care, and palliative care. 

 

The Fitch Unit provides care to inpatients and outpatients with oncologic and urologic diagnoses in addition to other medical/surgical conditions.  All of our patients are assessed and reviewed through an Interdisciplinary Team approach.  The disciplines that work collaboratively on this unit include:  Nursing, Food & Nutritional Services, Social Work and Case Management, Physical Therapy, Occupational Therapy, Speech Therapy, Pharmacy, Respiratory Therapy, Pastoral Care, Palliative Care Team and Integrative Medicine.  Our care delivery framework is the Elliot Model of Nursing, which supports these four basic tenets: Teamwork, Continuity, Competence and Caring.

We have created an environment that fosters open communication and professional relationships based on respect and equality for all.  The commitment to our profession can be seen in our care and dedication, our respect for confidentiality, and the fostering of nurturing and trusting relationships.  Through a system of primary care, we strive to help our patients as they decide how they will maintain their quality of life and regain an optimum lifestyle.  Our services extend to include families and friends by providing education and support.  Our continuum of care goes beyond diagnosis and treatment, and includes pain management, palliative and hospital services. 

 

Services Within the Unit:

Fitch Unit is primarily utilized to care for patients experiencing urologic and oncologic problems.   The inpatient and outpatient unit offers care to patients needing surgical treatment, chemotherapy, radioactive implant therapy, medical management, and palliative and supportive care in a caring and compassionate atmosphere.  Fitch outpatient services include blood transfusion, chemotherapy and rehabilitative services. 

 

The Palliative Care Unit provides a holistic approach to the care of patients and their families who are facing end-of-life care issues.  The goal of the Palliative Care Team is to provide comfort and support to this special group of patients and their families in hopes of improving the quality of their life in their final days together.   The cluster has been designed to promote a quiet, calm, and healing atmosphere.  Admission to the designated area is appropriate for any patient who has a life-limiting illness and is not receiving curative treatment, and for whom a short hospital stay is required for pain or other symptom management.  During the course of admission, members of the Palliative Care Team will work with the patient and family in order to facilitate a transition to an appropriate alternative setting (e.g., their own home, an extended care facility.) The team also provides teaching and support for the caregivers.

The Fuller Unit

Fuller Unit is a 36 bed Medical/ Surgical Unit on the eighth floor of Elliot Hospital. This unit has an eleven bed dedicated Neurology Cluster. Twelve beds are dedicated for General and Vascular Surgery patients. The remainder of our patient populations include: Respiratory, Diabetes, Renal or Nephrology, Ear, Nose and Throat; and most recently Bariatric Surgical Patients in collaboration with the physicians and staff of Dartmouth Hitchcock Clinic. All of our patients are assessed and reviewed through an Interdisicpiplinary Team approach. The disciplines which work collaboratively on this unit include: Nursing, Nutrition Services, Social Work and Case Management; Physical Therapy, Occupational Therapy, Speech Therapy, Pharmacy, Respiratory Therapy, and Pastoral Care. Our care delivery framework is the Elliot Model of Nursing, which supports these four basic tenets: Teamwork, Continuity, Competence, and Caring.

Fuller Unit staff have developed the following Vision: "Our dream is to make Fuller Unit a comfortable place to stay, where our staff want to give those little extras to create a warm environment that fosters friendships amongst staff and patients. Our academic achievements, common goals and our equalization of workloads will result in an innovative health care team. Our philosophy within the Elliot Model of Nursing will be demonstrated by our honesty, humor, trust, loyalty and respect for patients, peers, families, and those we serve."

Services Within Unit

Fuller Unit receives many arteriogram patients both pre and post procedure. The nursing staff on Fuller unit assess and monitor those outpatients who have had arteriogram procedures until they are stable, and their recovery period meets discharge criteria. This unit also manages outpatients who are scheduled for Plasmapheresis through the contracted services of Manchester Kidney Center.

Fuller Unit participates on a Hospital wide committee that evaluates and promotes coordination of care for all Ischemic Stroke Patients as well as patients who are admitted for the diagnosis of Transient Ischemic Attack. We have designed a pamphlet to assist patients and families in understanding stroke. It is available in the Emergency Department and on Fuller Unit: "Stroke Care at Elliot Hospital".

The King Unit:

King Unit is located on the seventh floor of the Elliot Hospital. It can accommodate 35 patients and features two designated nurse’s stations.  The unit has 15 semi-private rooms and 8 private rooms, family waiting rooms and conference rooms available for patient and family consultations and education.

 

All of our patients are assessed and reviewed through an Interdisciplinary Team approach.  The disciplines that work collaboratively on this unit include:  Nursing, Rehab Services, Patient and Family Services, Utilization and Quality Management, Pastoral Care, Home Care, and Food and Nutritional Services. Our care delivery framework is the Elliot Model of Nursing, which supports four basic tenets: Teamwork, Continuity, Competence and Caring.

Services Within the Unit:

King Unit is primarily utilized to care for patients experiencing orthopedic, neurosurgical, plastic surgery, gynecological surgery, respiratory care, antepartum and postpartum diagnoses. The ages of patient population range between 18 and 95+ years of age.

King Unit is open 24 hours a day, 7 days a week.  Visiting hours are from 12:00 p.m. to 8:00 p.m.  Children under 14 years of age are permitted to visit when supervised by an adult visitor.
