 Diabetes Education and Resource (DEAR) Program at Elliot Hospital

The DEAR Program at Elliot Hospital provides Diabetes Self Management Education (DSME) to people with diabetes. Our staff works closely with the physician to help their patients solve the “Diabetes Puzzle.” We provide assistance with blood glucose monitoring, medical nutrition therapy, medications and exercise, with the goal of preparing our patients to take charge of their disease.
Group Classes
This 9-hour education series cover a variety of topics important in successful diabetes self-management, allowing you and your family to have better diabetes control and to help prevent complications.
Session 1: Learning to Live with Diabetes
Types of diabetes, medications (insulin and other diabetes medications) and stress management
Session 2: The Basics of Eating
Effect of food on blood glucose, planning meals, carbohydrate counting, food label reading, eating out and the effects of alcohol
Session 3: Regulating Blood Glucose
Monitoring your diabetes, high and low blood sugar, sick day management, exercise and physical activity
Session 4: Putting it All Together
Personal health habits, long-term complications, behavior change
This set of 4 sessions is held twice a month at the following days and times:
Daytime classes: Mon/Wed/Thurs 9 to 11 a.m.; Tuesday 9 a.m. to noon.
Evening Classes: Mon/Wed/Thurs 6.30 to 8.30 p.m.; Tuesday 6 to 9 p.m.
Individual Consultation
The DEAR Program at Elliot Hospital also offers individual consultation with a nurse educator and/or registered dietitian. Families and/or significant others are included in all aspects of education. We are committed to providing quality education based on the American Diabetes Association guidelines.

- Blood glucose monitoring
- Medical Nutrition Therapy (MNT) by a registered dietitian
- Advanced carbohydrate counting
- Pre-pregnancy and pregnancy consulting for women with diabetes
- Gestational diabetes and its management
- Insulin instruction
- Prevention of Long-term diabetes complications
- Low blood sugar management/prevention
- Sick day management
- Behavior changes, goal setting, problem solving
- Setting goals for a safe exercise
- Relationship among eating, exercise, medication and monitoring
- Insulin pump program

About our Staff
The DEAR staff is comprised of nurses and registered dietitians who are certified in diabetes education. They have many years of experience and are actively involved in professional organizations committed to patient advocacy. They are dedicated to the advancement of patient education and are strong proponents of increased funding for diabetes research.

Enrollment
For appointments or more information, please call DEAR at 663-3130. Referrals should be obtained from your primary care physician. Self-referrals are also accepted.
Many commercial insurance companies, such as Cigna, Blue Cross/Blue Shield, Healthsource, Medicare and Tufts cover diabetes education. Call your insurance provider for details.
Education is free to family or friends involved in your care.
