This population includes EPN patients aged 18 and above with a diagnosis of either asthma or COPD. Our overall goal is to improve outreach, care, and influenza vaccination rates to our EPN patients with these diagnoses. Through improving education and follow up care with this population, we hope to achieve these goals.
