This population includes EPN patients ages 3-4 and 8-9 with a BMI at the 85th percentile or above. Our overall goal is to improve outreach and care to our EPN patients who fall under this category. Specifically, these patients will be receiving either nutritional counseling, referral to nutritionist, or educational material. They should also be receiving a follow up office visit 4 months after index visit.
Additionally, each EPN practice that treats this population has also received a copy of "A Guide to Creating Fit and Healthy Families - A Toolkit for Pediatric and Family Practices". This toolkit contains professional articles, educational materials and handouts, as well as age appropriate resources to help combat childhood obesity.
