Pediatric Well Child Visits Ages 0-12 Months and 13-24 Months
Our overall goal is to improve comprehensive preventative care among EPN patients. It is recommended that children from newborns to age 12 months receive 6 well child visits for routine care and follow up during their first year. We measure physician office performance for this group at 14 months.
It is recommended that children from age 13 to 24 months receive 3 well child visits for routine care and follow up during their second year. We measure physician office performance for this group at 30 months.
We have developed a set of 2 measurements that are monitored and reported on monthly. They are as follows:
