Senior Specialty Services Medication Management Clinic The Medication Management Clinic at Elliot Senior Health Primary Care offers patients the chance to meet with the clinical pharmacist one on one to discuss their medications. The pharmacist works directly with your primary care provider as part of your overall healthcare plan. The pharmacist will review your medications with you and will look for things such as drug interactions, side effects, unnecessary medications, potential cost savings, understanding of medications and how effectively you take your medications. The pharmacist can also work with you to help you meet your goals for blood pressure, cholesterol and diabetes management. Anticoagulation Clinic The Anticoagulation Clinic works one on one with patients to monitor and counsel patients for the duration of their anticoagulant therapy. During your appointment, a registered nurse performs a finger stick test, reviews you results with you and adjusts your dose appropriately. Your results are then updated to your electronic medical record where your physician can review them any time. The program monitors your prothrombin time (PT, INR) so that you will achieve and maintain desired therapeutic levels. A visit with the nurse requires a referral from your primary care provider. Podiatry Clinic The podiatry clinic is exclusively for the patients of Senior Health Primary Care. Patients with diabetes and other foot ailments would benefit from this clinic. For an appointment please call 603-663-7030.
TRACE Program™ Often the transition from one place to another can be stressful and difficult to manage, especially when the patient is focused on recovery. In addition, the patient may feel overwhelmed during this period of time away from home. Having support during transitions can improve the patient’s ability to manage transitions and achieve improved medical outcomes. A Coach may work with a patient on a one-to-one basis during the recovery process and through the care transition. The Coach can provide patients with the tools and support to take a more active role in managing their medical care. This Coach serves as a liaison and advocate for the patient however the coach is not a member of the patient’s healthcare team. The support that the patients receive maintains their involvement and improves their understanding of the healthcare system. The Elliot Health System now offers this support system for patients making health care transitions during their medical recovery.
