Patient Information

You can expect a comprehensive history and evaluation to review your current and prior level of function, home environment, and social supports to assist with discharge planning.

We encourage patients, when able, to bring in their own orthotics, prosthetics, and shoes to improve safety. We supply walkers, crutches, canes, other assistive devices, and adaptive equipment needed to mobilize in the hospital. We will assist you with obtaining new equipment if needed for discharge. If you have customized equipment such as a wheelchair, you are welcome to have it brought in for your personal use.

Therapists work with the patients to develop goals and a plan of care based on their specific needs. Discharge recommendations are made at the time of evaluation but may be modified based on progress during the hospital stay. Therapy works toward the highest level of independence a person can achieve while in the hospital. Recommendations for rehabilitation settings may be made based on your continuing needs and will be pursued with the entire treatment team, including the provider and case manager.

Patients are seen at least one time per week, with increased visits based on the therapy plan of care, discharge needs, and level of independence. Visits are scheduled electronically, however the dates/times are arbitrary and not reflective of actual session times.
