MOM (Maternal Opioid Misuse) Grant Program


Elliot Health System is proud to partner with the New Hampshire Department of Health and Human Services (DHHS) to implement the MOM (Maternal Opioid Misuse) Model. The MOM Model is grant-funded by the Centers for Medicare and Medicaid Services through 2024 and builds upon the Greater Manchester region's current opioid use prevention and treatment efforts for pregnant and postpartum women.
As the Care Delivery Partner working with the New Hampshire Department of Health and Human Services (DHHS), the Elliot has partnered with many community organizations, including:

  • Elliot Hospital
  • Catholic Medical Center
  • Amoskeag Health
  • Mental Health Center of Greater Manchester
  • Families in Transition
  • Farnum Center
  • Waypoint
  • The Doorway
  • Department of Health and Human Services
  • Department of Children, Youth and Family Services

The MOM program is available to women who are currently pregnant and up to two years postpartum, have opioid misuse disorder, and who are eligible for Medicaid. The program helps participants find a range of health and wellness services quickly and easily. By serving as an access point to these resources, the MOM program helps new and expecting mothers and their babies have good health during pregnancy and beyond.
The services that we provide access to include:

  • Medical, behavioral health, and substance use screening
  • Care coordination and case management, including housing assistance, service connection and collaboration, and food assistance
  • Behavioral health services
  • Substance use treatment, including outpatient, inpatient and residential, and recovery supports and resources
  • Perinatal and postpartum care, including OB/GYN connections, check-ups, birthing classes, midwife services, and lactation support
  • Family planning
  • Nutrition services
  • Medication Assisted Treatment (MAT) services
  • Peer support services
