Insurance Card, including your policy number and/or Medicare/Medicaid card.

Prescription Medication(s) a list of all medications, including over-the-counter medications, vitamins, and supplements, and the amounts you take.

Our practice requires a referral to be seen by one of our providers. To request a referral, please contact your primary OB/GYN provider or other healthcare provider.

You are allowed to bring one adult support person to this appointment. No children under the age of 18 are permitted to accompany you.

If you are in your first trimester, please have a full bladder for your appointment.
