Patient Information

You can expect a comprehensive history and evaluation. We will work closely with both you and your provider to develop an individualized plan of care to meet your recovery goals.

We recommend wearing loose, comfortable clothing and footwear.

All our visits are one-on-one treatment time, allowing you to develop a relationship with your therapist(s).

Insurance Card – including your policy number and /or Medicare/Medicaid card

Photo Identification

Co-payment is expected when you check-in. For your convenience we accept checks, cash, Visa, Mastercard, Discover, and American Express.

List of Prescription Medication(s) with dosage and frequency – only if not a current patient of The Elliot

Referral/Order – if not already received by The Elliot

Our goal is to ensure each patient is matched with the right therapists, at the most convenient location, as soon as possible.

At Elliot Rehabilitation Services, we have a team of highly-trained patient service representatives here to assist you with all aspects of scheduling your appointments. For your convenience, we offer flexible appointment times ranging from 7am – 7pm Monday through Thursday and 7am – 4pm Fridays. We have an outpatient network of seven different locations spread around the Southern NH/Manchester area, so there is sure to be an option that works with your busy schedule. You will need a therapy order from your referring provider.

SolutionHealth providers will electronically send orders on your behalf.

If your referring provider is outside of SolutionHealth, orders can be faxed to 603-663-8482, and we will call to set up an appointment. You are always welcome to call if you have an order in hand.

When scheduling your evaluation appointment, our patient service representative will gather all necessary insurance information. We will verify your benefits prior to the first appointments and determine if there are any authorizations or pre-certifications needed.

*We encourage all patients to contact their insurance company to verify benefits and any deductible information for their plan. We can be limited with getting deductible information.

*All Elliot Outpatient Rehabilitation departments bill as an “Outpatient Hospital Facility”.
