Patient Information

To schedule, cancel, or reschedule an appointment, please call our office at 603-663-6680 or log into your MyChart account. If you do not have a MyChart account, please click here to sign up.

If you need to cancel your appointment, please call our office at 603-663-6680 and allow 48 hours' notice so we may offer that appointment to another patient. Cancellations less than 24 hours before your appointment may be subject to a cancellation fee that is not payable by insurance.

Insurance Card, including your policy number and/or Medicare/Medicaid card.

Co-payment is expected when you check-in. For your convenience, we accept checks, cash, Visa, Mastercard, Discover, and American Express.

Prescription Medication(s) in their original bottles or a list of all medications, including over-the-counter medications, vitamins, and supplements, and the amounts you take.

Please arrive 15 minutes before your scheduled appointment.

Please allow 48 hours to refill medications.

Our physicians see patients 18 and older.

Any individual with a sleep complaint can be seen at the Elliot Sleep Evaluation Center. Many patients are referred by their physicians, but self-referrals are also accepted. The referring individual should call the Center to schedule an appointment. The first appointment will be a consultation, which includes an interview with a sleep specialist and a brief physical exam. During the visit, a decision will made if a formal sleep study is needed.

Diagnostic Testing, Treatment, and Follow-Up

Home Sleep Test (HST): is a sleep study done at home that records heart rate, oxygen saturation, respiratory airflow, and respiratory effort.

Polysomnography: (PSG) performed “in lab” at Homewood Suites with the attendance of a sleep technologist that records heart rate, oxygen saturation, respiratory airflow, and respiratory effort. This test generally requires a prior authorization from an insurance company.

Results of the diagnostic tests are interpreted by one of our board-certified sleep specialists. The patient may return to the Center for a follow-up test result appointment. The diagnosis would be discussed with the patient, and treatment recommendations would be made.

CPAP prescription orders are placed as applicable with an approved home medical equipment company.

CPAP Initiation & Management: An appointment may be scheduled with a sleep technician to assist newly diagnosed sleep apnea patients with compliance when using their CPAP device.
