Physicians and nurse practitioners at Elliot Senior Health Primary Care are able to follow your care in the event that you need additional recovery time or will be residing in select assisted living or nursing facilities.
These resources will help caregivers to care for themselves while helping loved ones.
Learn strategies for caring for yourself, identify ways to build a support system, and learn new coping strategies. Open to the public. Free and confidential. Held on the first and third Tuesday of each month, at 10 to 11:30 a.m. Contact Barbara MacPhee, MSW at 603-663-7051 for information. Co-payment is expected when you check in. For your convenience, we accept checks, cash, Visa, Mastercard, Discover, and American Express.
Prescription Medication(s) in their original bottles or a list of all medications, including over-the-counter medications, vitamins, and supplements, and the amounts you take.
