How to Make an Appointment

Your provider will take the following steps when referring you for an infusion or injection:

• Send authorization for treatment and treatment orders to Elliot Outpatient Center:
  • Providers from Elliot Health System or Southern New Hampshire Health will send orders through Epic
  • Providers from other hospitals and health systems will fax orders to 603-663-7609
• Call our staff to schedule the patient’s initial appointment at 603-663-7601.
• Ensure all required labs and paperwork are up to date before your appointment.
