Medication Management Clinic

Medication Management Clinic

Working with your primary care provider, an Elliot clinical pharmacist will review your medications with you for drug interactions, side effects, and potential cost savings. The pharmacist will provide you with education for the medications that you take, as well as education for conditions such as high blood pressure, high cholesterol, and osteoporosis.

Benefits

By adding a pharmacist to your healthcare team, you strengthen your ability to reach your treatment goals and reduce the risk of experiencing medication-related problems.

How It Works

Our pharmacist will look for things like:

  • Drug interactions
  • Side effects
  • Unnecessary medications
  • Potential cost savings
  • Understanding of medications
  • How you take your medications

If you have high blood pressure, high cholesterol, or osteoporosis, the pharmacist will work with you to help meet your therapeutic goals and provide you with education, monitoring, and follow-up on any medication changes that are made by your primary care provider.

Making an Appointment

Ask your primary care for a referral and call 603-663-7027 to schedule your first appointment. Please bring all your medications with you to your appointment so that the pharmacist can review the medications you take on a regular basis.
