All co-payments are expected at the time of service. For your convenience, MasterCard and Visa are accepted in addition to cash and checks as a method of payment.
Our billing office transmits electronically to all insurance companies. Patients are expected to provide the office with complete billing information, including mailing addresses and certificate numbers. Patients are responsible for their co-pays, deductibles and co-insurance.
If you are involved with a managed care insurance plan, please be sure to follow the guidelines of your plan. Be sure to choose specialists who participate with your insurance, and get written referrals when needed. We ask you to call at least a week in advance for any referrals you need. If you choose to go outside of your managed care program, with or without your primary care provider's referral, you may be responsible for the entire bill. We do consider patients responsible for their accounts. Please be aware of your health insurances' policies.
Please update our office of any personal or insurance changes that may affect processing of your claims or contacting you in case of emergencies.
We do understand that occasionally circumstances arise which necessitates special payment arrangements. Please contact our Billing Department to assist you between 8:30 AM and 4:30 PM Monday through Friday at 603-627-1669, then press #4.
If you need a prescription refilled, call 602-627-1669, then press #1 for our prescription line Monday through Friday 8:30 AM to 4:30 PM. Please allow 24 hours to process your refill request. We will only call you if there is a problem with the prescription.
