Our Diabetes Self-Management Education (DSME) programs will help you to become the team leader in your diabetes care. We work with your physician to help you find a balance between blood glucose monitoring, medical nutrition therapy, medications, exercise and prepare you to take charge of your diabetes.

  • Setting goals for a safe exercise
  • Relationship among eating, exercise, medication and monitoring

Consultations & Classes

Individual Consultation

Individual consultation with a nurse educator and/or registered dietitian with families and/or significant others is included in all aspects of education.

Diabetes Self-Management Education (DSME) Classes

Our nine-hour education series covers a wide range of topics relating to diabetes self-management. This information will enable you to confidently learn how to control your diabetes and prevent complications.

  • Class 1-On the road to better managing your diabetes
  • Class 2- Diabetes & healthy eating
  • Class 3-Monitoring your blood glucose
  • Class 4-Continuing your journey with diabetes
  • Class 5-Putting it all together

*All patients will start with individual DSME (Diabetes Self-Management Education) or MNT (Medical Nutrition Therapy) and our Diabetes Educator (CDCES) will work with patients based on their specific needs to determine if they will continue on an individual basis or join a group.
